What is Non-Incisional Double Eyelid (Buried Suture)?
Non-incisional or buried-suture double eyelid surgery (매몰법) forms a crease by threading permanent sutures under the skin through 2 to 3 pinpoint openings. No skin is cut away, so it suits younger patients with thin lids.
Swelling is usually modest after 3 to 7 days. The trade-off is durability: the fold can loosen or disappear over several years, especially in thick or fatty lids.
How it works
After marking and local anesthetic, the surgeon passes a fine nylon suture from the skin side to the conjunctiva and back, then ties it so the knot sinks below the skin. Variants include the continuous buried suture, the 'natural adhesion' or 'dot' methods, and partial-incision hybrids that remove a little fat through a 3 to 5 mm opening.
Who it suits
- Thin upper lids with little fat
- Patients in their late teens to 30s
- People who need a short recovery
- Those who want an option that can be reversed
Who should skip it or wait
- Heavy lids with a lot of fat or excess skin
- Drooping lids that need muscle correction
- Patients who have already lost two buried-suture creases
Side effects and risks
- Crease loosening or disappearing
- Suture knot showing as a small bump or cyst
- Irritation of the eye surface if a suture is exposed inside the lid
- Uneven folds
Aftercare
- Cold compresses for 2 days
- Avoid rubbing the eyes for 2 weeks
- Skip eye makeup for 3 to 5 days
- Report any scratchy feeling on the eye, which can mean an exposed suture
Planning around your trip
Stay at least 3 to 4 days for a check visit; there are usually no stitches to remove, and flying after 2 days is fine.

Non-Incisional Double Eyelid (Buried Suture) questions
How long will a buried-suture crease last?
Many last 5 years or more, but loss rates are higher for thick lids. Some clinics offer a free redo within a fixed period, so ask about the warranty terms in writing.
Can it be removed if I don't like it?
Yes, within the first weeks the sutures can usually be found and cut through a tiny opening. After the tissue has scarred in, removal is less predictable.
Can I go sightseeing the next day?
Most people do, with sunglasses. Swelling is highest on day 2 and is mostly gone by day 5.
What is the difference between 2-point and 3-point methods?
The number refers to how many fixation points hold the crease. More points spread the tension and may hold longer, but the right number depends on lid width.
Do I need stitches removed?
No. The sutures stay under the skin permanently, so there is no removal visit unless the clinic used an extra external stitch.
What happens if the crease drops later?
It can be redone with sutures again, but a second loss usually leads surgeons to recommend the incisional method.
