What is Under-Eye Fat Repositioning?

Under-eye fat repositioning (눈밑지방재배치) treats eye bags and the dark groove beneath them at the same time. Instead of just removing fat, the surgeon moves it down to fill the tear trough.

In younger patients with good skin tone it is done through the inside of the lower lid, so there is no external scar. If loose skin is also present, a lower blepharoplasty incision under the lashes is added.

How it works

Through a transconjunctival incision on the inner lid, the surgeon releases the fat pads and the ligament at the tear trough, then spreads the fat over the bony rim and fixes it in place with sutures. The inner incision is usually left to heal on its own or closed with dissolving stitches.

Who it suits

  • Puffy lower lid bags
  • A visible groove or shadow under the eye
  • People in their 20s to 40s with firm skin
  • Patients tired of repeating under-eye filler

Who should skip it or wait

  • Dark circles caused only by pigment
  • Heavy loose lower lid skin, which needs skin excision too
  • Recent under-eye filler still in place (it may need dissolving first)

Side effects and risks

  • Uneven contour or small lumps of fat
  • Persistent hollowness if too little fat remains
  • Lower lid pulling down (more likely with skin incision)
  • Prolonged bruising
  • Dry or irritated eyes

Aftercare

  • Ice for 48 hours, then warm compresses
  • Avoid bending, lifting and exercise for 1 week
  • Use the antibiotic eye drops as prescribed
  • Skip concealer for about a week

Planning around your trip

Stay 5 to 7 days for a check and any external stitch removal; bruising may still show on the flight but flying is safe after day 3.

Under-Eye Fat Repositioning questions

Is there a visible scar?

Not with the transconjunctival approach, because the incision is inside the lid. If skin is removed, a fine line sits just under the lashes.

How is this different from just removing eye-bag fat?

Removal alone can leave a hollow, aged look. Repositioning keeps your own fat as a filler for the groove.

I had under-eye filler last year. Is that a problem?

It can make dissection harder and mix with the fat. Surgeons often dissolve hyaluronic acid filler with hyaluronidase 1 to 2 weeks beforehand.

Why are some Gangnam prices so low?

Listed event prices may exclude sedation, tests or VAT, and often apply to first surgery only. Revision cases cost more.

How long will the bruising last?

Usually 7 to 14 days. Yellow tints can be covered with makeup from about day 7.

Can I combine it with lower lid skin removal?

Yes. If you have fine crepey skin, a small skin pinch under the lashes can be added, which means external stitches out on day 5 to 7.
