What is Revision Rhinoplasty?

Revision rhinoplasty (코 재수술) is more involved than a first nose job because scar tissue, missing cartilage and stretched or thinned skin all have to be managed. Common reasons include a visible or shifted implant, a short and upturned nose from capsular contracture, and a tip that is crooked or too pointed.

Many Gangnam surgeons who focus on revision use autologous rib cartilage or donated rib cartilage to rebuild support.

How it works

The old implant is usually removed along with the capsule around it. The surgeon then rebuilds the septal support and lengthens the nose with cartilage grafts. Where the skin is thin, dermis or fascia is added as a cushion. If infection is present, the implant is removed first and reconstruction waits about 6 months.

Who it suits

  • Shifted, crooked or visible implants
  • Short, upturned nose from contracture
  • Over-projected or pinched tip
  • Breathing problems after earlier surgery

Who should skip it or wait

  • Patients less than 6 to 12 months from their last nose surgery
  • Active infection (needs removal and waiting first)
  • Expectations of a perfect result from heavily scarred tissue

Side effects and risks

  • Warping of rib cartilage grafts
  • Infection
  • Skin damage in thin, scarred skin
  • Chest wall pain or scar at rib harvest site
  • Need for a further revision

Aftercare

  • Sleep on your back with the head raised
  • Keep the splint dry until removal
  • Support the chest when coughing if rib was taken
  • Avoid contact sports for 3 months

Planning around your trip

Plan 10 to 14 days in Seoul: nose stitches and splint come off around day 7, chest stitches around day 10, then you can fly.

Revision Rhinoplasty questions

How long should I wait after my first rhinoplasty?

Most surgeons want at least 6 to 12 months so swelling and scar tissue settle. Infection or an implant pushing through skin are exceptions that need early removal.

My own rib or donor rib?

Your own rib has lower infection and absorption risk but leaves a 2 to 3 cm chest scar. Donor rib avoids the scar but may absorb more over time.

Can my nose be made longer again after contracture?

Usually yes, but the skin limits how much length can be gained in one surgery. Expect a realistic change rather than the original goal.

Should I bring my old surgery records?

Yes. Implant type, graft materials and operative notes help the surgeon plan and quote accurately. A CT scan is often taken at consultation.

How long does a revision take to look natural?

Swelling lasts longer than after a first surgery. Plan on 3 months before judging the shape and 12 months for the tip.

Is the price fixed before surgery?

Usually a range is quoted, because the need for rib cartilage may only become clear during the operation. Ask what triggers the higher price.
