Forehead Augmentation (Rounded Forehead Surgery)

Surgery that gives a flat or sloping forehead a rounder profile, using an implant, bone cement or your own fat.

Quick facts
  • Korean name: 이마성형
  • Procedure time: 1 to 2 hours
  • Anesthesia: General anesthesia or IV sedation
  • Sessions: 1
  • Downtime: 7 to 14 days; swelling may move down to the eyes
  • Results: Shape visible at 2 weeks; settled at 3 months
  • Lasts: Implants and bone cement are permanent; fat partly reabsorbs

What is Forehead Augmentation?

Forehead augmentation (이마성형) is a Korean favourite for softening a flat or backward-sloping forehead, which is thought to make the side profile look younger and the nose bridge blend better.

Options range from fat grafting and filler, which add soft volume, to a pre-shaped or 3D-printed silicone implant or bone cement for a firm, permanent shape.

How it works

For an implant, the surgeon places a custom silicone shell beneath the forehead tissue through a small incision in the scalp, often with an endoscope. Bone cement (such as PMMA or hydroxyapatite) is shaped directly on the bone. The scalp incision is closed with staples.

Who it suits

  • Flat or sloping forehead
  • Depressions above the brows
  • Patients who keep reabsorbing filler or fat
  • Profiles that need balancing after rhinoplasty

Who should skip it or wait

  • Very thin forehead skin, where implant edges may show
  • Patients wanting a small, reversible change (filler is simpler)
  • Active scalp infection

Side effects and risks

  • Implant shifting or visible edges
  • Seroma (fluid build-up)
  • Capsular contracture around the implant
  • Scalp numbness
  • Infection needing implant removal

Aftercare

  • Wear the head band as directed
  • Avoid pressure on the forehead for 4 weeks
  • Sleep on your back with the head raised
  • Keep the scalp incision clean and dry for 48 hours

Forehead Augmentation questions

Silicone implant or fat graft?

Silicone gives a predictable, permanent shape and can be removed. Fat feels natural but may need two sessions because part of it is reabsorbed.

Is a custom 3D implant worth it?

A CT-based implant fits the skull closely, which reduces edge visibility and shifting. It costs more and takes time to manufacture, so plan ahead.

Can the implant be taken out later?

Yes, silicone implants can be removed through the same scalp incision. Bone cement is harder to remove.

Will my hairline change?

No. The incision is hidden behind the hairline and does not move it.

Why is no price shown?

Prices vary widely between custom implants, bone cement and fat, and we did not find a reliable published KRW range. Ask clinics for itemised quotes.

How many days of swelling should I expect?

Forehead swelling peaks at day 2 to 3 and often slides down to the eyelids. It is mostly gone by 10 to 14 days.
