What is V-Line Surgery?

V-line surgery (브이라인) combines mandible angle reduction with chin narrowing to turn a square or wide lower face into a tapered one. It is a bone operation done in a hospital setting under general anesthesia.

Incisions are inside the mouth, so there are no external scars. Recovery is longer than people expect: a soft diet for about 4 weeks and visible swelling for 4 to 6 weeks.

How it works

The surgeon cuts the jaw angle in a long curved line and shaves the outer cortex to reduce width. The chin is narrowed with a T-shaped osteotomy, removing a central bone segment and fixing the two sides together with plates and screws. Height and projection can be adjusted at the same time. Drains are often left for 1 to 2 days.

Who it suits

  • Wide, square jaw angles
  • Long or wide chin
  • Patients wanting a narrower lower face from the front view
  • Asymmetric lower jaw

Who should skip it or wait

  • Wide face caused mainly by masseter muscle (jaw Botox may be enough)
  • Bite problems that need two-jaw surgery
  • Thin, loose skin that may sag after bone removal
  • Uncontrolled bleeding disorders

Side effects and risks

  • Numbness of the lower lip and chin from mental nerve stretch
  • Asymmetry or step-off at the jaw line
  • Soft tissue sagging after bone reduction
  • Bleeding or hematoma
  • Rare: unplanned fracture or nerve injury

Aftercare

  • Wear the compression band as told, often full time for 1 week then at night
  • Liquid then soft diet for about 4 weeks
  • Rinse the mouth after every meal
  • Sleep with the head raised

V-Line Surgery questions

Will my lip be numb afterwards?

Temporary numbness of the lower lip and chin is common and usually improves over 3 to 6 months. Permanent numbness is uncommon but possible.

What can I eat in the first month?

Liquids for the first days, then soft food such as porridge (juk) and tofu. Most surgeons allow normal chewing after 4 to 6 weeks.

Will my skin sag after the bone is removed?

Patients in their 20s usually adapt well. Over 35, or with thin skin, some surgeons add a lifting procedure to reduce sagging.

Do I need a CT scan with V-Line Surgery?

Yes, a 3D CT is standard so the surgeon can see the nerve canal and plan safe cuts.

When can I fly home?

Most clinics clear patients 10 to 14 days after surgery. Flying earlier is not advised because of swelling and bleeding risk.

Is jaw Botox an alternative?

Only if the width is mostly muscle. Botox slims the masseter but cannot change bone shape or chin length.
